To book the South Dallas Concert Choir for a performance within the Dallas metropolitan area, please complete this Community Artist Program (CAP) application from the Dallas Office of Cultural Affairs.  Requests should be made at least 4 weeks in advance of the requested performance date.  The form must be completed and emailed to the Artist Resource Coordinator, Rhonda Ivery, at rhonda.ivery@dallascityhall.com (please copy us on your request using SouthDallasConcertChoir@GMail.com).  If you’d like more information regarding the Dallas Office of Cultural Affairs Community Arts Program please visit their website at: Community Artist Program or if you have questions specific to requesting an artist, please contact Rhonda Ivery at (214) 671-8573 or the South Dallas Concert Choir at (469) 609-7322.

When completing the form, information about your organization goes in sections 1 and 2.  Section 3 is where you make the request and describe what type of performance you would like from the South Dallas Concert Choir.  There are blocks where you can request multiple performances, but all you need to complete is the first block where South Dallas Concert Choir is listed as the Artist/Group.
